Inquiry into Royal Oldham Hospital baby death

The Pennine Acute NHS Trust (PAT) has ordered an independent inquiry into the death of baby Thomas Beaty following his inquest last week.  A previous inquiry, which concluded in January, investigated the deaths of five other babies and three mothers at the trust between December 2013 and last July.  It concluded that the deaths did “not appear to be the result of deficiencies in care” as the women had health complications. However, in some cases, risk management during the antenatal period and in labour was “below standard” and that there had been a “notable absence of clinical leadership in both medical and midwifery teams, resulting in a failure to adequately plan care”.
